General Notice (Full Version)
For employees who have been "involuntary terminated" on or after February 17th thru December 31st 2009.

General Notice (Abbreviated Version)
For employees that have been “involuntary terminated” but already have Cobra coverage.

Notice in Connection with Extended Election Periods
For employees that have been “involuntary terminated” from September 1st 2008 thru February 16th 2009.

Alternative Notice
For employees who are eligible for IL state continuation.
